CP Unlimited operates an extensive community based living program, day habilitation programs, and community habilitation services. CP Unlimited focuses on giving people the personal, social, and vocational supports needed to live in their communities. Care coordination is provided to assist persons with disabilities and their families in gaining access to service and support appropriate to their needs. CP Unlimited also provides rehabilitation, nursing, vocational services, behavioral, dietary and Article 16 services including counseling, physical therapy, occupational therapy, and speech therapy.
Currently CP Unlimited is seeking a full time Eligibility Specialist to join our team of professionals to prepare Medicaid and Social Security Administration (SSA) recertification’s; completing representative payee and Supplemental Nutrition Assistance Program (SNAP) forms/application packets; ensuring timely redemption of SNAP benefits.
Specific responsibilities include
- Verifies accuracy of information provided to ensure maximization of expected benefits.
- Processes initial applications and annual recertification’s of Supplemental Nutrition Assistance Program (SNAP) for all residents in congregate care level II facilities.
- Redeems monthly SNAP for each CC level II resident and compiles summary by borough.
- Assist in completing NYC & NYS Medicaid recertification for all CC level II and Intermediate Care facility residents and delivers to HRA office.
- Assist in preparing Social Security recertification for all residents receiving Social Security Income.
- Prepares Social Security representative payee forms for all residents living in our programs.
This is a key position requiring an Associate’s Degree, 2 years’ experience preferred, Proficient with Microsoft Office which includes Microsoft Excel.
