Crandall Medical Center, a large long-term skilled nursing and rehabilitation facility is hiring for a part time Licensed Social Worker (LSW). Crandall Medical Center is located in Sebring Ohio and ranked by Newsweek as one of the top Nursing Homes in Ohio. The Licensed Social Worker reports directly to the Administrator and is a 24 hour a week non-exempt position.
The Licensed Social Worker:
- Meets medically related social services needs of residents and maintains compliance with Federal and State Nursing Home regulations.
- Performs many duties assisting residents with all levels of mood and behavior assessments
- Completes Care Plans
- Participates and coordinates the Level and Plan of Care Meetings
- Works with residents and family members upon admission and discharge
- Works with Physicians on monitoring resident mood and behaviors

Professional Experience/Educational Requirements:
- Bachelor's degree in social work or related field.
- One (1) year experience in health care social work preferred.
Certification / Licensure:
- Current Ohio Licensed Social Worker
