Position Title: Admin Care Coordinator Supervisor: CEO
Job Summary:
Develops and conducts behavioral health and alcohol and other drug abuse prevention and training programs and provides technical assistance to drug abuse service providers. Admin Care Coordinator manages the intake process of the mental health programs and serve as a liaison with other mental health providers, referral sources, local and state governing bodies, as well as other Creative Pathways, Inc. programs. Admin Care Coordinator assists in implementation of an integrated service delivery system with other Creative Pathways, Inc. programs and various community organizations. Represents Creative Pathways, Inc. at all required meetings.

Duties/Responsibilities:
- Gathers and distributes educational materials to increase individual, family, and community participation in addiction programs and trainings collects and evaluates data.
- Conduct face-to-face contact, video- conferencing, or telephone, communicates treatment needs, assessments, and treatment information to external health care providers and facilitates appropriate linkages with other health care professionals, including transitions into or from higher levels of care or institutional settings.
- Organizes plan of care implementation activities that are separate from the diagnostic assessment services.
- Assist with community planning processes including data collection and analysis, determination of capacity, preparation of issue plans, implementation of research-based strategies, and evaluation of outcomes.
- Adopts a “whole person” approach to address the client’s needs related to physical health, behavioral health, and social determinants of health
- Coordinates care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital).
- Reviews client behaviors or indicators common to client success and premature failure; recommends strength-based approaches that facilitate successful treatment outcomes.
Required Skills/Abilities:
- Able to research, compile, analyze, interpret, and disseminate information.
- Competency to develop, conduct, and evaluate educational and training programs.
- Ability to demonstrate sensitivity to cultural differences as they relate to program and service delivery.
- Able to prepare reports and maintain records; interpret regulations; and evaluate compliance with prevention/treatment service provider contracts.
- Ability to establish and maintain working relationships with colleagues, contractors, clients, and community groups; ability to communicate clearly and effectively in a variety of situations including public forums; and ability to work in a collaborative environment.
Educational Requirement: A high school Diploma or GED minimum. Master’s Degree is preferred in mental health related field.
Licensure and/or Certification: First aid and CPR certification. Valid state driver’s license, proof of auto insurance and ability to utilize personal car for travel within the region.
Experience: At least three year’s experience, to include demonstrated leadership, familiarity with operational, financial, quality assurance, and compliance related operations of the organization. Knowledge of DC regulations. Teamwork oriented, flexibility in working assignments and locations.
