We are a 4 star-Medicare certified and Joint Commission accredited home health agency licensed to serve 9 counties of the state of Illinois since 2008. Crescent works closely with acute and post-acute care facilities all over Chicagoland to coordinate care for their patients upon discharge.

This is a wonderful and exciting opportunity for a licensed RN/LPN to provide excellent care to our patients in Elgin and surrounding areas. This is a per diem position, with no On Call required!

Responsibilities and Duties

The Case Manager, under the direction of the Supervisor, is responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, and provision of direct nursing care within the policies and standards of the Agency. The Case manager makes the initial patient evaluation visit except in circumstances where the physician had ordered therapy only and the Supervisor approves admission by the Therapist.

Qualifications and Skills

1. RN License in the state of Illinois

2. Minimum five (3) years of clinical experience with at least half of that time working in home health or related health care organization

3. Plan, organize and manage assigned patients.

4. Supervise home health aide where applicable.

5. Knowledge of OASIS E and Axxess software is a plus

6. Sound understanding of Medicare home health care standards, regulation, guidelines, QA and compliance is a plus-we will provide excellent training

Pay: $65.00 - $120.00 per hour
