Bilingual Social Services Care Coordinator (English / Spanish)

Substance Use Disorder Treatment Facility

BASIC FUNCTION

• Coordinate clients’ ancillary health needs, focusing on clients’ primary medical care and financial assistance services

RESPONSIBILITIES

• Ensure clients’ TB tests are completed within 7 days of admission
• Apply clients for state disability benefits, as needed
• Apply clients for Medi-Cal or My Health LA benefits
• Arrange for client transportation to medical facilities
• Assist clients to enroll with a primary care physician/medical group
• Schedule new clients for physical examinations
• Maintain professional rapport with all networking agencies
• Act as liaison between clients and CDWs concerning medical issues
• Act as liaison with Health Advisor for medication approval
• Monitor clients ongoing medical issues and clients’ medications
• Review all medical treatment forms when clients return to the facility
• Report all client illnesses to Program Manager
• Inform the Program Manager of all client accidents, injuries and medical issues
• Update and ensure accuracy of clients’ medication records
• Document case management in Sage
• Perform other related duties as assigned by the Social Services Manager

• Excellent computer skills
• Strong organizational, follow-up and time management skills
• Proficient in Microsoft Excel
• Ability to work well within a team structure
• Excellent oral and written communication skills
• Professionalism
• Bilingual (Spanish / English)

EDUCATION / TRAINING

• If the individual is a registered addictions counselor, they must be enrolled in an academic Addiction Studies Program

LICENSE / CERTIFICATION REQUIREMENTS

• Position requires individual be registered and/or certified with one of the state-approved addiction counselor certification entities (i.e. CADTP, CAADE, or CCAPP) or be a Masters level BBS registered individual

SPECIAL CONDITIONS

• Must have a valid California driver's license
• Must be willing to undergo random drug screenings

WORKING CONDITIONS

• Occasional ascending and descending stairs
• Frequent sitting for prolonged periods
• Occasional crouching below the waist and / or above the shoulders
• Occasional operation of motor vehicles
• Occasional work may be performed at various locations across Los Angeles County

Job Type: Full-time

Pay: $23.00 - $29.00 per hour
