Position Summary
The Registered Nurse is responsible for ensuring quality and safe delivery of Hospice services and is accountable for coordinating provisions of Nursing services reflecting the Hospice philosophy and standards of Crossing Rivers Health.


Essential Job Functions

  • Implements and complies with current nursing practice, following department approved policies and procedures as well as State and Federal guidelines.
  • Continually evaluates the nursing needs of the patient, redirecting, and offering suggestions as necessary.
  • Observes signs and symptoms of patients’ condition and promptly reports to the provider any significant changes.
  • Completes point-of-care documentation to ensure efficiency.
  • Directs and supervises quality care given by Certified Nursing Assistants offering guidance and direction as necessary.
  • Relays information to patients, families and Hospice members regarding costs for Hospice.
  • Provides information regarding equipment and supply needs of the patient.
  • Participates in the provision of nursing care that includes direct patient care, according to the needs of the patient following Interdisciplinary Team care plan.
  • Understands and abides by the Hospice Philosophy, the phases of death and dying and is knowledgeable of the phases of grief and bereavement.
  • Recognizes physical, emotional, social, and spiritual needs of the patient and family, delegating team members appropriately.
  • Promotes and provides education for staff members, patients, families, volunteers, and the community,
  • Utilizes equipment, supplies, resources and time in an effective and efficient manner, ensuring budget compliance and cost effectiveness.
  • Effectively contributes to the establishment of priorities based on the needs of the total family.
  • Participates in on call duty to provide 24 hrs/day; 7 days a week nursing coverage to Hospice patients.
  • Participates in continuing education as determined by the department.
  • Demonstrates knowledge of State Licensure, Federal Certification and Accreditation requirements, as well as state and federal regulation and agency policy.
  • Other job duties and responsibilities as assigned to effectively meet the needs of the patients, the department, and the organization as a whole.

Requirements

Education

  • Associate’s Degree Required, Field of Study: Nursing
  • Bachelor’s Degree Preferred, Field of Study: Nursing

Experience

  • Two plus years of experience in Nursing preferred
  • Knowledge and experience in Hospice preferred

Computer Skills

  • Basic Computer Proficiency

Certifications & Licenses

  • CPR
  • Valid Driver’s License
  • RN licensed to practice in Wisconsin and Iowa

Other Requirements

  • Reliable vehicle
