The Hospice & Palliative Medical Social Worker assists patients and families by providing resources for care to enhance the patient's quality of life during adjustment, planning and after end of life care.

Position Qualifications:

  • 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care or Acute Care environments
  • Licensed Master's Level Social Worker Required
  • Medicare guideline experience preferred


Job Responsibilities:

  • Assess and document patient/family social, emotional, financial and psychological needs as well as resources on the Comprehensive Assessment
  • Assist patient and/or family with establishing DNR, advance directives and/or DPOA or Healthcare proxy as requested
  • Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, GIP and as dictated by patient/family needs
  • Work in both Hospice and Palliative care Programs
  • Office is located in the Akron/Canton area and travel to the office is required.


Shift:  Monday through Friday 8:30 AM to 5:00 PM with an on call rotation that includes evening and weekend hours
