Position Title: Case Manager
FLSA: Exempt
Qualifications:
- BSW or BA, BS, in Human Services, Psychology, Sociology or related fields of study
- At least three years of experience working with at risk youth.

Summary of Essential Duties: To advance ethical practice and professionalism while supporting protection emotional stability and healthy childhood development through service coordination, linkage, advocacy, etc.
- Compile all relevant information and documentation pertaining to youth
- Participate in service coordination on behalf of youth and families, under supervision of the MDT and/or licensed therapist
- Work in tandem with the licensed therapist, youth, family/guardian and all other members of the MDT providing care coordination input in the development of individual service plans, treatment plans and IMDS tools
- Serve as School, Community, and Ancillary liaison.
- Represent the youth and family on the agency's behalf as it relates to screening, placement, court
- appearances, etc.
- Remain in consistent contact with all members of the youths MDT (internal/inhouse and external)
- Provide youth-centered advocacy
- Provide referral and linkage to ancillary and or specialized treatment services and interventions
- Routinely conduct school, home, and ancillary visits
- Schedule and attend special appointments (medical, allied, etc.)
- Responsible for correspondence on behalf of the youth and family-treatment team meeting invitations, as well as monthly/quarterly plan dissemination to members of the MDT
- Responsible for youth file management
- Participate in treatment conferences, individual educational plan, child family team, emergency review meeting etc.
- Actively participants in, MDT, House Rounds, Clinical Rounds and Record Reviews
- Responsible for PerformCare authorization status and ensuring Medicaid coverage eligibility
To document accurately and effectively all clinical services provided.
- Documents all weekly contacts on behalf of youth and family
- Documents using clear, concise language that accurately represents case management efforts-service linkage and coordination.
