Job Summary:
- Review home care and hospice SN, AIDE, MSW daily notes for accuracy as needed.
- Analyze data to present quarterly QAPI data for home care and hospice.
- Provide feedback to home care and hospice supervisors for needed QA improvements.
- Attend education classes, IDT, and case conference as needed.
- Assist hospice and home care teams as needed in day-to-day operations.
- Provide one-on-one education/QA review with appropriate clinicians.
- Demonstrate and encourage team behavior and exceptional patient/guest experiences.
- Uphold and promote patient safety and quality.
Qualifications
Education:
License in practical nursing or associate degree in nursing required.
Experience:
Minimal 2 years’ experience as home care field nurse required. Preferred experience in hospice. Preferred hospital med/surg; ER or critical care experience.
Additional Skills/Abilities:
Leadership skills; great communication skills; critical thinking skills; knowledge in use of Microsoft and Excel; flexibility and ability to multitask and prioritize. Must be organized and driven to complete tasks by a deadline.
