About the Position:
Our clients do not predominantly require nursing care. The Client Advocate RN is responsible for identifying educational needs for clients and staff in areas including but not limited to diagnosis, vitals monitoring, BG monitoring, daily weights, medical equipment, medications, and wounds. This role involves collaborating with the client's medical team, identifying opportunities for improving environmental safety, and assessing the client's health status and trends to maintain optimum health.
Pay:
- Starting Pay is $40 per hour, with possible increase after 90-day introductory period and successful completion of a Professional Development Plan
Key Responsibilities:
- Engage in active health management practices including monitoring vitals, reconciling medications, and coordinating with medical teams.
- Educate and support both clients and staff on best health practices, equipment usage, and safety improvements.
- Conduct monthly health reviews and maintain accurate health documentation.
- Serve as a liaison between clients, their families, and health professionals to ensure cohesive care delivery.
- Address health-related queries from staff by providing education and recommendations as needed.
Requirements:
- Active California RN license.
- Current BLS certification.
- Proven experience in nursing, preferably with exposure to a community or outpatient setting.
- Strong interpersonal skills with a knack for effective communication and relationship-building.
- Commitment to maintaining HIPAA and confidentiality standards.
