What are the responsibilities and job description for the Population Health Coordinator Sr position at Dartmouth-Hitchcock Health?
Overview
With minimal support from supervisors, coordinates multi-stakeholder internal and/or external partnerships and project teams to plan and implement population health initiatives.
- This role supports our statewide perinatal quality improvement initiatives with Alliance for Innovation On Maternal Health (AIM)
- Data collection and analysis to inform quality improvement efforts
- Remote and on site support for clinical practices implementing improvements based on the AIM initiative.
- Hybrid role that requires time in Lebanon, NH and some travel as needed around the state for on site data collection and support.
Responsibilities
Qualifications
- Bachelor’s degree
- Minimum 4 years’ experience in related field.
- Excellent interpersonal and organizational skills, creativity, flexibility, diplomacy, Allyship and the ability to work as part of and lead teams with positive team culture.
- Demonstrated ability to handle multiple projects under pressure with minimal support, required.
Required Licensure/Certifications
- Holds or obtains field-specific and dept. certifications within 24 mos.
- Trained in basic facilitation/ leadership of teams, RBA, and DEIB/Equity/Allyship
