Salary Range:    Undisclosed

Summary:

This position reports to the Support Supervisor and is responsible for the intake/financial information for each client being accurate and entered into the BUI system as well as assisting clinical staff with placement of clients into residential or assisted living facilities, as well as facilitating Community Support Services for clients. FBC is a 24-hour Residential Service, open 365 days a year. Potential for full-time, part-time or, PRN (on-call as needed), based on facility need.

  • Responsible for ensuring that financial information, including initial interaction with insurance companies, for the clients is completed correctly and entered into the CMHC system for reimbursement.
  • Responsible for assisting clinical staff with placement of clients in the appropriate facility as determined by their needs. Determines clients’ eligibility for requested facilities.
  • Works with residential facilities, halfway houses, and shelters to refer appropriate clients and secure placements.
  • Maintains spreadsheet for LME of requested data regarding referral and placement of clients.
  • Responsible for obtaining signed releases from the clients to allow the release of required information to the appropriate facilities/referrals.
  • Requests re-authorizations from LME for clients staying longer than the initially authorized time, logging into the appropriate spreadsheets
  • Requests authorizations for any walk-in clients
  • Attends shift reports relaying information as to who is scheduled to leave and what arrangements have been made
  • Keys daily SALs

Facilitates referrals to Community Service Providers and makes outpatient appointments for Daymark outpatient services

  • Coordinates and logs monthly fire drills on rotating shifts
  • Maintains Facility Referrals notebook with information on any new referral agencies and providers of MH/SA services

  • Responsible as back-up for front desk in the absence of staff

  • All other duties as assigned by supervisor or other management

Education and/or Experience: Must possess at least a High School Diploma. Office experience in the healthcare field preferred.
