What are the responsibilities and job description for the Medical Transporter position at DDMS LLC?

FPDC is searching for a Medical Transporter to provide assistance to our residents while at medical appointments. This position will serve as an integral member of the Program and Medical Teams and will assist with the day-to-day activities of the facility. This is a M-F position but the hours may vary throughout 6:00 am to 5:00 pm, depending on the appointments scheduled.
Responsibilities
• Assures the Individual(s) are prepared for appointments and transports them in a safe, effective manner in accordance with
any physical management plans to appointments at the designated date and time.
• Assures that all necessary medical records, paperwork and forms accompany the individual to all appointments.
• Communicates with the medical staff at the appointment to provide required information.
• Upon return to the facility, reports information from appointment to the appropriate medical staff and submits all medical
forms from appointment to Health Services Specialist or assigned staff.
• Communicates regularly with the HSD and other appropriate staff regarding an Individual's problems or progress.
• Assists and supports individuals in performing daily living functions in all areas that require individual personal assistance.
• Assist and supports individuals in all housekeeping duties.
• Monitor individuals and the surroundings for appropriate safety and health care practices.
• Assist, coordinates and transports individuals on recreational activities in their homes and in the community.
