Direct Support Professional

Do you enjoy working in a residential home supporting adult individuals with intellectual and developmental disabilities while being responsible for the daily operations of the residential group home? Delta Projects is looking for Direct Support Professionals for a Respite home in Wrentham, MA.

The ideal candidate will:

• Follow Protocols initiated by Delta Projects and DDS for individuals and the program
• Maintain professionalism with co-workers, management, and stakeholders at all times
• Take individuals to urgent/emergency/routine medical and dental appointments and provide support during these appointments and communicate with team on updates from these appointments
• Attend to the daily needs of individuals on an individual basis as well as foster increased independence and self-determination
• Administer and document individual medications as prescribed by a physician in accordance with agency and program procedures
• Record and appropriately utilize necessary data, daily progress notes, staff log and complete all other ISP and program required documentation before the end of shift
• Transport individuals to and from medical appointments and other activities as required
• Provide opportunities for leisure time activities and foster independence in the choice and participation of those activities.
• Ensure safety and comply with program standards for staff coverage.
• Provide a supportive environment that includes opportunities for individuals to determine, to the fullest extent possible, personal preferences for daily schedules and routines.
• Communicate in a professional manner.
• Ensure a clean, safe and home-like environment for clients.
• Participate in core training and maintain certification of required trainings including but not limited to CPR, First Aid, and MAP.
• Provide crisis intervention and access emergency services as needed.
• Implement all programs for individuals, which may include physical interventions such as lifting and transferring individuals.

QUALIFICATIONS:

• High School Diploma or equivalency
• Good communication skills- ability to communicate well with individuals, co-workers, team, stakeholders, and providers.
• Ability to maintain professionalism & exemplify dignity and respect, especially in high stress, adaptive environment.
• Adaptability to frequent changes & various needs of individuals and program.
• Certified Nursing Assistant (CNA), Personal Care Assistant (PCA), or Home Health Aide (HHA) certification preferred
• Direct care experience working with Acquired Brain Injury population preferred
• A valid driver’s license in state of residence
• Qualifying criminal background and driving record
• Must possess a safe, reliable, registered, insured, and inspected vehicle in order to transport individuals as needed/required during the course of assigned shift(s)
• Must be able to demonstrate competence in certain areas of training including but not limited to CPR, First Aid, physical management, behavior management, medication administration, and driving
• Ability to recognize inappropriate or manipulative behavior in difficult or challenging situations
• Ability to calmly and repetitively model appropriate behavior in difficult or challenging situations
• Ability to react to situations, i.e., rescue, preventing a resident from hurting themselves
