General Description

Under direction of the Director of IDD Services and within federal, state, and local guidelines performs routine work meeting the needs of individuals with developmental disabilities. Work involves assessing and determining individual needs, developing, and monitoring of Person Directed Plans, and connecting individuals with services and supports.

The IDD Enrollment Specialist will serve as the Local IDD Authority's HCS and TXHML enrollment coordinator. Responsible for transition planning activities for people leaving Nursing Facilities and State facilities; which includes interaction with NF, SSLC, and State Hospital staff. Work involves independent judgment, good interpersonal skills, computer skills, and travel within the service area. The position maintains a varied caseload of persons with multiple, complex needs. Job responsibilities are conducted in accordance with the Texas Administrative Code (TAC), HHSC Performance Contract, HHSC program specific Handbooks, Medicaid requirements, Center policies and procedures as well as other applicable state and federal laws. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

This position has the ability to work partially remote once all training is complete and productivity measures are consistently met.

Education, Training and Experience (including licensure and certification)

This position requires a bachelor's degree from an accredited college or university; or an associate degree in a social, behavioral, human service, or health-related field, including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice; or a high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.

At least one-year human services experience is preferred. LIDDA or Service Coordination experience is preferred.

REGISTRATION, CERTIFICATION, LICENSURE OR OTHER QUALIFICATIONS:

  • All job specific training must be completed within 30 days of hire.
  • Must maintain a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

  • Knowledge of programs for individuals with Intellectual and Developmental Disabilities and community resources
  • Skill in the operation of computers and in applicable computer software.
  • Ability to independently organize and prioritize a variety of assignments while managing time effectively.
  • Skilled in professional, effective communication, both orally and in writing.
  • Desire to monitor and advocate for individuals in services.
  • Skilled in effective problem solving and conflict resolution.
  • Ability to work in a non-judgmental manner with individuals whose values and belief systems may be different from the employee.

Essential Duties

  • Completes assigned HCS and TXHML enrollments in the designated timeframes as regulated by HHSC.
  • Provides on-going services to assist individuals with Intellectual and Developmental Disabilities, or related conditions, in developing a Person Directed Plan (PDP) that identifies desires outcomes and services.
  • Meets with clients face to face throughout the enrollment process to provide ongoing monitoring and assistance to the client and family.
  • Documentation of face-to-face contacts and collateral contacts during the enrollment process.
  • Completes Diversion Plans, Transition Plans, and Pre-Move Site Reviews.
  • Completes progress notes within three business days.
  • Serves as advocate for individuals by reviewing rights initially and as needed, informing individuals/LARs of the Center's complaint process and promoting individual choice.
  • Assists individuals with applying for Medicaid for program eligibility.
  • Works collaboratively with HCS and TXHML program Providers to facilitate enrollment.
  • Performs data entry on the Enrollment tracking spreadsheet.
  • Submits enrollment extensions request to HHSC in accordance with the HHSC Performance Contract, ensuring 95% compliance in this area on a quarterly basis.
  • Maintains positive relationships with individuals, family members, service providers, Center employees, other agencies and the community.
  • Returns phone calls and e-mails in a timely manner. Follows up with individuals and/or issues in a timely manner.
  • Completes all required documentation as required and in accordance with Center policies and procedures, HHSC contract, Medicaid requirements, applicable program handbooks, and the TAC.
  • Responds to individual crisis situations when they arise, ensuring the health and safety of the client, and making appropriate reports to entities within the centers designated timeframes with minimal supervision
  • Works both independently and as part of a team.
  • Independently schedules and makes client contacts.
  • Perform additional duties as deemed necessary by department supervisor within designated timeframes.
  • Ensures that all required documentation is in each individual's medical record chart.
  • Provides back up to other Service Coordinators during vacancies or caseload capacity issues.
  • Carries a varied, temporary, caseload during program vacancies as requested.
  • Provides Service Coordination in compliance with applicable TAC and Performance Contract requirements.
  • Ensures that enrollment activities are performed in compliance with applicable Texas Administrative Code, LIDDA Handbook, and Performance Contract requirements.
  • Completes ICAP's, ID/RC Assessments, IPC's, and PDP's for enrollment purposes.
  • Performs data entry into to the Texas Medicaid & Health Partnership (TMHP) portal,
  • Utilizes and maintains access to Client Assignment and Registration System (CARE) database and any other database as deemed necessary.
