A provider (nurse practitioner or physician assistant) who works under the direction of the Supervisor of Advanced Practitioners in Cardiology to improve outcomes and workflow in the Cardiology Clinic and Population Health and Prescription Management Department.

- Assume direct primary patient care of patients referred by Clinical Services or as assigned by Medical Director-Clinical Services.
- Endorse and practice proactively DOHC values in all interactions with patients, outside clients and employees.
- Attend staff meetings, promote departmental and organizational goals.
- Coordinate care with case managers as requested by Clinical Services.
- Assist other DOHC departments to achieve group and corporate goals such as HCC and 5 Star, Patient Experience.
- Participate in Quality and Utilization Review activities.
- Patient charting to be performed on Electronic Medical Record (EMR) as per time frame specified.
- Maintain clinical practice standards.Be courteous, flexible and adaptable.
- Maintain patient confidentiality and understand Compliance such as HIPPA and FWA.
- Interact and communicate effectively, with Medical Directors, Providers, Senior Managers, Supervisors, and other DOHC/FHC personnel verbally, electronically and otherwise.
- Interact and communicate effectively with patients and families, including leading family meetings in end-of-life and hospice discussion.
- Share the regular and holiday Clinical Services on-call schedule as determined in rotation with other providers. Call included both telephonic patient management as well as evaluation of patients.
- Prove coverage to other providers in other programs, including scheduled patient contact in clinic, home or SNF.
- Non-patient contact time will be solely determined by the Medical Director, or delegate.
- Perform other duties as assigned.
- As applicable, possession of State of California RN license, Advanced Degree in Nursing/Advance Practice Nurse Certification, for Nurse Practitioners or graduate of an approved Physician Assistant Program as outlined in the California Code of Regulations and possession of unrestricted Physician Assistant license by the Medical Board of California, for Physician Assistants.
- Active DEA number and Furnishing Number.
- Comprehensive experience in primary care and/or Cardiology.
- Knowledge of Managed Care.
