Full–Time Employee Salary Annual Range: $26.00 - $32.00
Key Responsibilities
- Case Management:
- Coordinate with counselors, Residential Reentry Center (RRC), and medical providers to ensure comprehensive treatment appointments.
- Schedule clients to receive appropriate resources and services.
- Schedule client intakes and necessary medication appointments.
- Manage client medication processing.
- Manage treatment-resistant clients by effectively explaining authorized services.
- Participate in team meetings.
- Communication:
- Serve as a primary point of contact for clients
- Facilitate communication between clients, RRC staff, and service providers.
- Communicate regularly with clients to assist with the coordination of authorized services.
- Documentation and Record Keeping:
- Maintain detailed client records and documentation.
- Assess weekly paperwork timelines to ensure currency and compliance with the Statement of Work (SOW).
- Monitor the submission of weekly schedules to relevant business partners.
- Monitor the referral system, updating any rejected or missing items as needed.
- Compliance and Invoicing:

- Coordinate annual Bureau of Prisons (BOP) monitoring.
- Ensure compliance with audit-identified processes and documentation requirements.
- Check and correct invoices weekly to prepare the monthly invoice.
- Prepare and submit a monthly invoice.
Qualifications
- Education:
- Substance Use Certification (required).
- Experience:
- Minimum of two years of experience in case management, counseling, or correctional settings.
- Knowledge of correctional systems, rehabilitation practices, and reentry strategies.
- Experience with Electronic Health Records (EHR) systems.
- Knowledge of evidence-based practices in rehabilitation and reentry planning.
- Skills:
- Strong interpersonal and communication skills.
- Ability to work effectively with diverse populations.
- Proficiency in case management systems and reporting tools.
- Ability to maintain confidentiality and handle sensitive information appropriately.
- Demonstrated ability to multitask and work independently and as part of a team.
- Strong problem-solving and critical-thinking skills.
- Proficiency in Microsoft Office and relevant case management software.
- Exhibit strong leadership qualities to support the team and clients.
- Other Requirements:
- Must pass a federal background check and obtain security clearance.
- Strong ethical standards and a commitment to confidentiality.
Working Conditions
- Must be able to work flexible hours and respond to urgent matters as needed.
- Provide counseling services on an occasional basis.
