Seva Home Health is looking for LVN’s for our growing Home Health business serving Madera and Fresno Counties. Mileage is reimbursed and driving time is paid.

Primary responsibilities will be:

• Serving as a point of contact throughout the admissions process
• Ensuring coordination of care with other team members and marketing team
• Communicating effectively with patients, family members, case managers, healthcare providers and co-workers
• Providing education to patients and family members regarding their home healthcare experience
• Providing an outstanding admissions experience for our patients

Job qualities for this role include:

• Receive and appropriately document new referrals in a timely manner.
• Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team.
• Provide effective communication to patients, staff members, other health care professionals and referral sources.

• Understands and applies a smooth and efficient admissions process and an outstanding experience for our customers
• Is able to multitask and has excellent organizational skills
• Has outstanding communication skills (written, verbal and non-verbal)

Requirements:

• 2 years LVN experience, home health preferred
• LVN license
• Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
• Experience with Kinnser (WellSky) EMR preferred
• Experience with Allscripts preferred
