Responsibilities for Home Health Manager.
- Direct the coordination of client care and caregiver task
- Monitor and follow up the clock in and clock out of caregivers in system
- Find coverage for call ins. If unable to find coverage, you are to cover the shift.
- Update case management notes, documenting change of aid, change of service, telephone complaints.
- Assist in training caregivers in client homes
- Assist in home visits/telephone follow up with clients
- Communicate with clients/families on home health related issues and report information to Administrator
- Performance of other duties as required
- Monitor clients and chart medical records accurately
- Report the condition of a client back to Clinical Manager and doctors
- Assist in Supervising nursing assistants
- Ensure that supplies, medications, and equipment are restocked and maintained so that they are ready to use
- Admit, discharge, and transfer clients
