Position Summary:
We are seeking caring, compassionate, mission driven Intensive Case Managers to include assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, entitlement, and vocational rehabilitation. The case manager would be working in a program called Treatment Works, which was created as a result of the Parkland School Shooting, to work with students that have been identified by Pinellas County Schools as presenting a threat to themselves and/or others. It includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual and their family, which reflects strengths and self-identified goals. The case manager will be working on an integrated decision making team including a clinician, an interdisciplinary facilitator, and members of Pinellas County Schools to address progress made on goals, safety concerns, and needs of the individual and family. As an integral part of case management services, discharge/transition services are arranged or coordinated by the case manager as an individual is preparing for discharge and transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training or school attendance.) All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs. Services provided are intensive, and community based to best meet the client and their family's needs.
Responsible to meet client facing client care requirements of a minimum of 26 billable hours per week.

EDUCATION EXPERIENCE/REQUIREMENTS:
Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with high risk k-12 students and their families.
Must be vaccinated against COVID-19 or be willing to be vaccinated against COVID-19 immediately upon hire. This is a condition of employment, exemptions to this requirement must be addressed in advance with HR.
