DISC Village, Inc. is a private non-profit behavioral health organization that has been operating in and throughout the Big Bend area of North Florida for over 50 years. Providing expertise in services in the areas of substance abuse, mental health, and child welfare to adults, adolescents, and families.
We are looking for a Dependency Case Manager to serve in our Gadsden County office.
Dependency Case Management Services: DISC Village Inc., strives to provide the highest quality of child welfare prevention and intervention services to children and their families within their communities of Wakulla/Franklin Counties and Gadsden/Liberty Counties. Our program is designed to provide a continuum of services that assist families with acute or chronic conditions that threaten child safety and family stability.
Essential Responsibilities:
- Meet all Florida statute requirements for children in the Dependency System
- Manage assigned caseload with children and families in child welfare system
- Supervise families and children on case load and make on-going assessments regarding child safety
- Complete assessments within required time frames
- Provide case management services to clients in accordance with established guidelines, regulations, policies and procedures
- Conduct home visits with children, parents and caregivers to assess for child safety, case permanency and well-being
- Rotational on-call duties
- Assist with court hearings through participation in hearings and arranging transportation for client/family
- Assist with alternative placements for children in home as necessary
- Maintain documentation for client records and input data into the Florida Safe Families Network (FSFN) as required
Qualifications:
- Bachelor’s degree from an accredited college or university in social work, psychology, mental health, or related field.
- Obtain Child Welfare Certification within probationary period and maintain on an ongoing basis required
- Ability to be sensitive to the service population’s cultural and socioeconomic characteristics
- Must be able to work under tight schedules and possess the ability to handle pressure associated with deadlines
- Valid Florida state issued driver’s license and insurance required
- Must have the ability to work under pressure and maintain self-control in the face of hostility or provocation
- Must possess verbal and written communication skills
