What are the responsibilities and job description for the Human Services Coordinator I position at DMR-Metro Region?
The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Human Services Coordinator I (HSC I) position. The Human Service Coordinator I is the primary contact for individuals and families served by the Department and are expected to effectively represent the Area Office by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department
HSC I responsibilities include, but are not limited to, the following:
- Coordinates and facilitates the development of the Individual Support Plans in accordance with DDS regulations and individual needs that reflect the individual’s vision, choices, and needs.
- Provides coordination and support to individuals enrolled in traditional as well as self-directed services: Participant Directed Program and Agency with Choice. Identify issues and concerns, then document steps taken to resolve issues.
- Monitors the safety and well-being of assigned individuals and the implementation of services provided by DDS through regular contact and site visits.
- Provides information and education for individuals, their families and guardians regarding DDS services arranged by the Area office, DDS contracted Family Support Centers, and generic community services. Provides information regarding application for state plan services and federal entitlement service programs for people with disabilities including Supplemental Security Income, Adult Foster Care, Personal Care Assistant, MassAbility, etc.
- Ensure individuals receive appropriate services. Arrange for referrals of service and support, and ensure individuals receive appropriate services both within DDS’s service delivery system and though generic support services, other state agencies, and community service providers.
- Monitor situations when individuals are at risk, work proactively, and when appropriate arrange for crisis intervention for individuals as needed: assessing needs and making recommendations for follow up.
- Monitors follow through on team recommendations through regular and consistent contact with individuals, families, and service providers while maintaining accurate individuals’ records and electronic records in Virtual Gateway and Meditech and maintaining confidentiality.
- Strive to establish and maintain harmonious working relationships.
- Collaborate closely with Area Office staff and provide information to the Area office management team.
- The HSC I will arrange and/or provide crisis intervention for individuals as needed; assessing needs and making recommendations for follow up planning, as well as providing information to supervisor and other members of the Area Office Management Team. This position will perform other related duties which may include individuals engaged in Participant Directed Services.
Please Note: This is Full-time, 37.5 hour position.
Shift: 8:45am - 5:00pm
Days Off: Saturday and Sunday
Flexibility required to meet the needs of the Area Office.
