We are seeking a dedicated and organized individual to join our team as a Housing Coordinator within the Health and Social Services (HSS) sector. As a Housing Coordinator, you will play a crucial role in ensuring that individuals within our community have access to safe and stable housing, particularly those who are receiving health and social services support. Your responsibilities will include managing housing resources, collaborating with service providers, and advocating for the housing needs of vulnerable populations.
Client Assessment and Coordination:Conduct comprehensive assessments of clients' housing needs, taking into consideration their health and social service requirements.
Collaborate with healthcare professionals and social workers to develop tailored housing plans for individuals and families.
Resource Management:Identify and maintain a database of available housing resources, including affordable housing units, supportive housing programs, and temporary shelters.
Work closely with property managers and landlords to secure suitable housing options for clients.
Advocacy and Networking:Advocate for the housing rights of clients within the HSS sector, collaborating with community organizations, government agencies, and other stakeholders.
Establish and maintain relationships with housing providers and community partners to expand housing options for HSS clients.
Documentation and Reporting:Maintain accurate and up-to-date records of client interactions, housing placements, and progress towards housing stability.
Prepare regular reports on housing activities, outcomes, and challenges for internal and external stakeholders.
Crisis Intervention:Respond promptly to housing crises, providing immediate assistance and support to clients facing homelessness or unsafe living conditions.
Collaborate with crisis intervention teams and emergency services to ensure the safety and well-being of clients.
