What are the responsibilities and job description for the PRP Counselor, Full Time/Part Time position at Eagle Physical Therapy Rehab?
PRP counselors render individual and group services to adults in the home, community, and on-site environment. Counselors are required to conduct 6 visits a month which includes one visit a week for an hour. Counselors are responsible for implementing, documenting, and monitoring client progress towards their individualized rehabilitation plans.
Counselors help to rehabilitate clients in the following domains:
Self-care skills--hygiene, nutrition, self-administration and compliance with medication;
Social skills--community integration activities, interactive skills with peers and authority figures, developing natural supports, and supporting the child's participation in community activities;
Independent living skills--maintenance of personal living space, community awareness, mobility skills, money management, and accessing available entitlements;
Cultural and spiritual activities--
Behavioral/Emotional self-management--conflict resolution, anger management, time management, maintaining age-appropriate boundaries.
Promotion of illness self-management--education and information about mental illness, effective strategies to assist in managing illness, warning signs of relapse, helpful interventions.

Counselors work in collaboration with all pertinent parties in the client's life, i.e. clinician, psychiatrist, DORS counselor, etc. All services delivered and data collected must be documented in the appropriate forms in a timely fashion. Counselors are the ones who teach and enrich the client lives by also sharing their individual interests, talents and life experiences.
Education Requirement: High School Diploma and related experience in the field.
Training and Certification requirements as deemed necessary by program:
- DLA 20 certification needed
