The Care Navigator will work closely with a Clinical Pharmacist Practitioner and Coordinating Registered Nurse and primary care teams at the practices to care for our patients. As a Care Navigator, you will serve as a patient advocate and be a liaison to help manage and improve patient outcomes and support the CM team with administrative and clinical tasks. An ideal candidate is a critical-thinker who is highly organized, patient-centric, and self-driven. Our Care Navigators are on the phone with patients throughout the day. They know how to build relationships by asking thoughtful questions and utilizing effective listening skills. They are responsible for clearly documenting patient interactions within Electronic Medical Record Systems (EMR) and company documents, so strong interpersonal, computer, and communication skills are needed. This position requires confidentiality, discretion, and exceptional customer service. 3-5 years of CMA / LPN clinical experience is strongly preferred. This is a remote position. Qualified candidates must be local to Greensboro. Must be able to work in office as needed as directed by supervisor.
Schedule: Monday - Friday 8 AM - 5 PM, NO WEEKENDS

Eagle Physicians may pay all support staff employees hired as a .50 FTE or higher on-or-after April 1, 2024, structured bonuses over their first two years of employment with Eagle as outlined provided. Bonus Amounts are $750.00 at 6 months, $750.00 at 12 months, $1,000.00 at 18 months and $1,000.00 at 24 months.
Paid Maternity/Paternity leave– Eagle provides up to 2 weeks of paid maternity/paternity leave after 1 year employed and 1,250 hours worked for the birth or adoption of a child. Time is pro-rated based on FTE status.

Key Responsibilities:
• Maintenance of therapeutic goal records for chronic care patients.• Coach patients as documented in their care plan to improve disease management, education etc.• Communicating with Manufacturer's medication assistance programs on behalf of patient and/or provider• Prioritize patient and CM team outreach and tasks based on risk stratification and level of need.• Understand chronic disease management as part of a multidisciplinary team.• Identify SDOH and access resources database or identify community resources to address these needs.• Understand and review quality metric reporting and gap lists.• Prepare provider summaries for the PharmD to review prior to the patient’s appointment.• Call patients for appointment reminders and follow-up assessments.• Documentation of intervention and care plan within the Electronic Medical Record (EMR) software.• Prepare full prescriber list for each patient.• Record patient's primary and secondary pharmacy insurance.• Assessment of potential coupon, voucher, or other discounts available to the patient for medications.• Coordinate medication alignment as instructed by pharmacist as well as preparation of dispensing history.• Maintain a positive attitude and connect with patients.• Monitoring for and notification of emergent or new prescriptions• Management of patient relationship in collaboration with clinical team, including frequent telephone interaction with patients• Other associated tasks that may arise from time to time at the instruction of the Clinical Pharmacist Practitioner• Demonstrates awareness of patient & visitor fears and apprehensions.• Participates in professional development activities and maintains professional affiliations.• Shows respect, courtesy and advocacy in all dealings with all people, including, but not limited to patients, coworkers, and visitors.• Displays a positive attitude, and team-oriented spirit, indicated by a willingness to assist where and whenever needed.• Attends meetings as required.• Writes legibly and with correct grammar and sentence structure.• Follows HIPAA guidelines for patient confidentiality and maintains security of Protected Health Information (PHI) and business information. • Does not exhibit, divulge, copy or remove the contents of any record or report except to fulfill work assignments.• Participates in professional organizations as indicated.• Upholds and exemplifies through daily actions, activities and behaviors Eagle’s Vision, Mission and Values:o Vision: Eagle Physicians will be seen as the healthcare organization of choice by patients, staff and the community.o Mission: Eagle Physicians will provide patient-centered healthcare for our community with a team committed to high-quality, supportive and compassionate care.o Values: Integrity, Compassion, Trust, Professionalism and Respect.• Upholds and exemplifies through daily actions, activities and behaviors Eagle’s Diversity, Equity, and Inclusion values. • Performs additional duties as required.
PERFORMANCE REQUIREMENTS:
Knowledge, Skills & Abilities
Knowledge of medical terminology to communicate with clinical staff. Skill in preparing written detailed documentation and input data into computer system. Skill in establishing and maintaining effective working relationships with patients, medical staff and the public. Ability to read and understand oral and written instructions. Ability to follow written protocols. Ability to work independently. Ability to communicate clearly.
Education:
• Medical Assistants: Graduate of an ABHES (Accrediting Bureau of Health Education Schools) or CAAHEP (Commission on Accreditation of Allied Health Education Programs) accredited program for Medical.• Licensed Practical Nurse: Graduate of an accredited associates degree or bachelor’s degree LPN program.
Experience: 3-5 years clinical experience strongly preferred. Certified Health Coach preferred. An equivalent combination of education and experience may be considered.
Certificate/License:
• Medical Assistants: Certification from the AAMA (American Association of Medical Assistants) or Registration from the AMT (American Medical Technologists). Eagle does not accept certification or registration from any other entity.• LPN: Unrestricted North Carolina Nursing License
