Title: Realignment Lead Case manager
Department: Rapid Rehousing
Reports to: Coordinated Entry for Singles and Housing Retention Manager
FLSA Status: Non-exempt
Directly Leads: Housing Specialists (2 FTE’s)
Salary: $65,000 to $70,000
SUMMARY
The Realignment Case Manager is responsible for coordinating case management and program services for an assigned caseload of realignment single adults and families who are homeless or at risk of homelessness. This position works closely with and leads two Realignment Housing Specialists in providing housing and community resources for each client household.
This position establishes a network of services and support that will enable clients to increase self-sufficiency, obtain and retain housing and avoid recidivism. The Lead Case Manager is responsible for connecting and coordinating with each client’s Probation Officer and the services offered by the Department of Probation to assist formerly incarcerated citizens.
The Realignment Case Manager is expected to adhere to the highest standards of service in the performance of their job assignment at all times. The individual working in this capacity models behaviors for their team and clients to promote a positive, professional attitude towards Realignment singles and families, staff members, and collaborative partners.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Service Delivery
- Enrolls 90% of DPO referrals into the program within 30 days of receiving referral.
- Provide a $20.00 gift card incentive (flex fund) to all new referrals to promote enrollment within 30 days of receiving referral.
- Assesses households for appropriate services and housing placement
- Reviews and approves assessments and service plans for Housing Specialists
- Assesses participants for strengths and barriers, including employment, behavioral health, etc. and refers program participants to appropriate services
- Develops and monitors service plans that identify specific actions needed to successfully stabilize households
- Educates clients about budgeting, financial management, including support with filing tax returns, filing for Earned Income Tax Credit, banking and credit repair
- Refers clients to all appropriate public benefit programs. Assists with collecting documentation and provides transportation to appointments as necessary
- Schedules and maintains regular meetings with clients (at least semi-monthly)
- Collects income verification and other documents needed to secure housing
- Recommends appropriate temporary financial assistance to the Housing Retention
Manager.
- Regularly reports client progress to Housing Retention Manager and discuss any recommendations
- Completes In House Standardized Intake, Realignment supplemental form, Service Transaction and Exit forms for HMIS. Submits completed forms to the Data Specialist within 24-hours of for entries and exits
- Update and maintain the drop box and ensure that it is current and accurate at all times
- Respond to all emails from HCD regarding drop box or HMIS issues
- Maintains case files in accordance with Realignment grant and EOCP guidelines
- Actively participates in case conference and program meetings
- Attends EOCP internal meetings and outside trainings as assigned
- Provides coverage for Transition Day Reporting Center 4 hours per week
- Provides outreach at Santa Rita Jail up to 4 hours per week
- Provides coverage for after-hours call-line for four months of the year
- Performs other duties as assigned
Supervision
- Supervises, guides two Housing Specialists on a weekly basis
- Provides weekly group supervision meeting to coordinate, review cases and conduct file audits to ensure client needs are met
- Conducts written performance evaluations on all supervisees annually
- Works closely with the Director of Training to ensure that all supervisees have a training plan and team members are being developed
- Ensures benchmarks are attained
Community Collaborations
Responsible for forming and maintain ongoing partnerships with Probation Officers and outer-agency service providers that will assist Realignment singles and families in the following resources:
§ Homelessness Prevention Coordination
§ Substance abuse and mental health interventions
§ Employment agencies, including, but not limited to BOSS, Center for Employment Opportunities (CEO), Private Industry Council (PIC), East Bay Works
§ Housing Authorities and other affordable housing agencies
§ Oakland Unified School Districts, when applicable
§ Head Start, when applicable
§ Department of Social Services - Cal-Works, when applicable
§ Bananas and/or other daycare providers, when applicable
§ Children’s Hospital and/or other health care providers, when applicable
§ Additional resources as applicable
Administration
- Gathers statistical data to monitor program effectiveness
- Works closely Quality Assurance Manager to ensure that data is collected accurately, analyzed and incorporated to enhance program impact
- In coordination with Quality Assurance Manager ensures that files meet agency and funding standard for completeness and content quality
- Maintains an accurate account of participants’ progress, via BIRP case file progress notes.
- Assists with providing input for grant reports
- Reports regularly to Housing Retention Manager on program performance

EDUCATION AND EXPERIENCE
- Bachelor of Arts degree in social work, psychology or related field required;
- 3-5 years’ experience working as a case manager with high risk, low income families, experience working with single Realignment and families preferred;
- Experience working with individuals re-entering from the criminal justice system and those from diverse ethnic backgrounds;
- Knowledgeable about dynamics of homelessness, families, substance abuse, mental health, domestic violence and parenting.
- Knowledge of basic counseling and interviewing techniques;
- Knowledge of rapid re-housing techniques preferred;
- Excellent organizational and time management skills;
- Demonstrated ability to maintain organized case management files;
- Ability to produce well-written reports, memos and other correspondence in a timely fashion;
- Ability to work effectively with a team of other professionals and coordinate services/activities with each member, as needed;
- Demonstrated ability to maintain a calm demeanor and communicate effectively with residents (verbal and written);
- Ability to use conflict-resolution skills to de-escalate volatile situations with clients.
- Strong self-awareness and sense of limitations. Appropriate professional and personal boundaries with high integrity.
- Ability to show flexibility and respond to the needs of families.
LANGUAGE SKILLS: English, second language, especially Spanish or American Sign Language a plus
MATHEMATICAL SKILLS: High School level
REASONING ABILITY: Able to utilize good judgment, think logically and make sound decisions. Demonstrated ability to outline realistic service plans.
COMPUTER SKILLS: Experience working with computers and successful utilization of Word and Excel. Must possess knowledge of the internet and be able to retrieve resources and email.
CERTIFICATES, LICENSES, REGISTRATIONS: California Driver’s license and insurance
PHYSICAL REQUIREMENTS: Must dress neatly and professional when reporting to work. This position requires an annual tuberculosis test.
