- Current certification as a Registered Nurse or Licensed Practical Nurse is required.
- Prefer experience in a medical setting.
- Individual must demonstrate mature judgment, be energetic, flexible and self-motivated with strong attention to detail, communication, teamwork, customer service and computer skills.
- Requires the ability to use clinical assessment skills and perception to determine the needs of the patient while working with the medical provider.
- Must maintain current CPR for all life cycles and CEU credits required for licensure.
- Must demonstrate excellent internal and external customer service skills.
- Employee must demonstrate an understanding of the intent, and follow standards, guidelines and protocols related to achieving and maintaining certifications by outside agencies.
- Must demonstrate a personal and professional commitment to Eastern Shore Rural Health and its Mission Statement.
- Ability to provide bilingual patient care is a plus.
As part of the Patient Centered Medical Home Model of care, the employee will advocate for patient’s best interest and make recommendations for patient special needs such as interpreter services; identified barriers to care such as transportation, medication assistance, outreach, and health education; influence patient’s engagement in their care; promoting a continuum of safe, quality, and cost-effective patient care.
- Expected to proactively collaborate and integrate with all members of the teams to achieve optimal care for the patient in the Medical Home model as designated Charge Nurse.
- Must be able to perform all essential functions of a nurse.
- Responsible for overseeing the functions and duties of assigned nursing staff, Pharmacy Assistant staff, Lab Technicians and X-ray staff, and provide input towards their evaluations.
- Ensures staff assigned to Center Nurse Manager (CNM) successfully attain their goals.
- Responsible for evaluating patient flow pertaining to the nursing aspects, and make appropriate adjustments and recommendations to the CNM.
- Assists in developing agenda for staff meetings and huddles.
- Assumes the responsibility of CNM in their absence, except in performing disciplinary action.
- May perform fact finding pertaining to occurrences in the absence of the CNM and gather statements for review upon their return.
- Assumes responsibility for daily scheduling/team assignments and overview of all nursing duties performed by teams.
- May serve as backup for immunization tracking and ordering. Orders medical supplies with knowledge of formulary/340B ordering.
- Reinforces updated clinical changes/requirements to ensure necessary adaptations are implemented.
- Assists in pulling clinical metrics on Clinical Support Staff (CSS) on a quarterly basis. Able to direct clinical training to best meet goals set by CNM with assistance from the Clinical Nurse Educator.
- Serves as Dickson One alarm backup.
- Serves as a preceptor and assists CNM in assigning team preceptor for newly assigned employees.
- Serves as clinical “validator” in Performance Manager for 30/60/90 and annual competency skills checkoff. Works closely with Clinical Nurse Educator on annual competency skills check off.
- Assists in monthly EOC as directed.
- Escorts patients to exam room and prepares them for examination by provider.
- Obtains nursing intake as listed in the procedure manual to include measuring vital signs, documentation in electronic medical record and initiating standing orders as appropriate.
- Provides patient/family education regarding vaccines and may recommend care-coordination opportunities when situation presents itself.
- Prepares for and assists provider with procedures prn (e.g. sutures, toenail removal, pelvic exam, splinting).
- Administers and documents vaccines, medications and treatments including allergy shots as ordered by provider and within scope of practice. Able to determine state versus private vaccines.
- Performs specimen collection (e.g. phlebotomy, capillary draws, and catheterized and clean catch urine) and CLIA waived labs according to our lab competency and document accordingly.
- Performs DOT and non-DOT drug screens.
- Performs technical procedures (e.g. EKG, hearing & vision) according to competency.
- Prepares specimens and orders for send out labs per lab formulary.
- Takes telephone messages and provides feedback to patients/providers/pharmacy calls and documents in electronic medical record.
- Must follow proper Triage procedure per policy utilizing the electronic protocols.
- Performs mentor duties to visiting clinical staff and new employees per Worksite Checklist.
- Attends and participates in all nursing in-service educational programs.
