What are the responsibilities and job description for the Community Health Worker position at Easterseals MORC?
We are looking for Game Changers :
Community Health Worker will work to promote, maintain, and improve the health of persons served. This position will work closely with medical providers, care teams, and other agencies to improve care outcomes. They will facilitate access to community services by connecting and helping persons served navigate systems of care. Specific duties may include:
- Establishes trusting relationships with persons served as they work collaboratively and effectively with clinical care teams.
- Collects patient medical histories and records vital signs.
- Meets regularly with the care team to plan care to discuss cases and exchange appropriate information with team members.
- Acts in advocacy and liaison role between persons served and community service agencies.
- Assists persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services.
- Assists with the completion of required information such as enrollment, eligibility determination and overall follow-up.
- Records and tracks care management information. Assesses information to identify patterns and/or trends.
- Maintains knowledge of community resources to assist in meeting the needs of persons served.
- Provides referrals for community services as appropriate.
- Assists persons served in the development of health management plans and personal care goals. Provides education on the effective management of their chronic health conditions and self care. Motivates individuals to be active and engaged in their health care.
- Implements wellness and prevention initiatives.
- Facilitates health education groups.
- Participates in community outreach activities.
- Skilled at listening, engaging, and responding to individuals at various states or readiness for change; those with complex issues; those in crisis.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist consumers with trauma related issues.

All other duties that are general in nature and are related to the scope and practice for this position.

Salary : $35,300 - $44,600
