What are the responsibilities and job description for the ACT Team RN Case Manager position at EastRidge Health Systems?
The Act Team Registered Nurse (RN) is responsible for providing high quality care in the community and outpatient settings to ACT team consumers. The ACT Team RN is expected to function well as a member of a multidisciplinary Clinical Team. The ACT Team RN is responsible for utilizing comprehensive skills in assessment, treatment planning, care coordination, case management, medication administration, crisis intervention and the documentation of services provided in direct care of ACT team consumers. The Act Team RN will also provide training and instruction to staff to ensure that the highest quality of care is provided to the consumers in such areas as, but not limited to, AMAP training, First Aid/CPR, Bloodborne Pathogens, and OSHA requirements (Tuberculosis Awareness instruction and annual screening). All services provided are to promote client independence and empower clients to manage their symptoms to insure an acceptable level of daily functioning.
Professional Registered Nurse licensed in the State of West Virginia. One year experience in Behavioral Health required. Valid Driver’s license with clean driving record required. Twenty-four hour accountability and responsibility for nursing services is required as needed to ensure the highest quality of consumer care and safety at all times.
