Overview
We are seeking an Adult Day Program Coordinator for our new Skilled Nursing Facility in St. Paul, MN! The Adult Day Program Coordinator is responsible for assisting the Director in the efficient operation of the program. Encouraging socialization and assisting in planning and implementation of programming needs of individual participants and groups. Responsible for ongoing charting and case management.

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well.
Responsibilities/Job Description
- Set up participants’ charts per licensing requirements
- Insure that all the forms are completed by family and staff and updated as necessary
- Maintain monthly charting; quarterly care plans and health updates
- Document monthly health education, fire drills, blood pressures and weights for each participant
- Responsible for preparation, implementation and clean up of daily programs i.e. crafts, woodworking, games, music, etc.
- Responsible for providing snacks and noon meal to participants. Set up , cutting up food, feeding participants as necessary and clean up
- Assist staff in developing monthly calendar
- Responsible for daily menu planning and ordering of meals and snack supplies
- Performs routine personal care and grooming at the center (including baths)
- Responsible for assisting identified participants with all toileting needs
- Promote and encourage individual care
- Provide individual and family counseling for participants as needed
- Communicate with staff the participants needs and individual plan of care
- Observe, assess and intervene where needed wit participants health concerns
- Reports any problems, progress and observations to the Director
Qualifications
Required
- High School Diploma/GED
- Current Driver's License in good standing
- CPR certification and Basic First Aide Training
Preferred
- Vocational/Technical Training or Associate Degree
- At least 1 year of experience
