Clinical Case Manager-LCSW/LMSW

The Clinical Case Manager performs comprehensive psychosocial assessments and formulates diagnosis of social, emotional and substance abuse problems, develops treatment plans for each client; monitors plan to ensure its implementation; and educating the client regarding wellness, medications, and health care compliance. The Clinical Case Manager serves as an advocate for the client. The Clinical Case Manager ensures, mental health, substance abuse, and other client services are maximized as indicated by diagnosis or clinical needs. Reports to the Chief Clinical Officer.

 

ESSENTIAL FUNCTIONS

  • Maintains a regular and predictable work schedule.
  • Carries a caseload from 20 to 30 behavioral health patients
  • Communicates with others (internally and externally) to provide, exchange, or verify information, answer questions and address issues of clients.
  • Accomplishes assessments and diagnosis to determine the need for social treatment establishes treatment goals and selects appropriate social work treatment techniques, based on problems and needs of clients and families/caregiver(s), which will be most effective.
  • Develops specific clinic or program treatment goals such as
    • developing communication patterns with members of other disciplines, and
    • determining priorities for services and the method of services (whether in areas of direct services, consultation, or participant teaching.
  • Objectivity and self-discipline to avoid emotional involvement in situations that is frequently emotional and high-charged.
  • Keeping abreast of new knowledge and techniques related to the practice of social work/clinical case management and new medical treatment modalities as they might affect the social adjustment or lifestyle of clients via literature, professional settings, and staff development activities.
  • Documentation of services provided in client record or other database(s).
  • Performs quality management/assurance activities.
  • Other duties as assigned.

OTHER RESPONSIBILITIES /ACTIVITIES

  • Participates in El Centro’s compliance, quality assurance, performance improvement, risk, and safety activities.
  • Other duties as assigned.

 

EDUCATION AND EXPERIENCE

  • Preferred Licensed Clinical Social Worker (LCSW)
  • Must have at least a (LMSW)
  • Must have had two years volunteer or paid in the provision of clinical social work services to vulnerable populations including individuals with behavioral health issues/concerns.

Qualifications


  • Bilingual in English/Spanish (written and oral)
  • Must have a current Texas licensure LCSW or LMSW

  • Excellent written and verbal communication skills
  • Strong understanding of social media best practices and trend
  • Ability to multitask and prioritize tasks in a fast-paced environment
  • Attention to detail and strong organizational skills

Salary : $75,000 - $110,000
