What are the responsibilities and job description for the Advantage Case Manager position at Elder Care?

We are hiring a Case Manager to join our Advantage team. Candidate must have one year of experience providing direct care with the aging population and either a bachelor’s degree or LPN/RN license. We offer flexible scheduling, mileage, and a great team environment. Sign on bonus for current Advantage Case Manager Certification!

We will consider training the right person with a bachelor’s degree or active nursing license. We serve: Washington, Nowata, Craig, Osage, Ottawa, Mayes, Delaware, Rogers, and Kay Counties and will work with you to keep you close to your home area. Ability for daily in area travel is required.

RESPONSIBLITIES:

  • Adherence to all agency policies and Advantage regulations
  • The assessment of client/member needs and capabilities
  • The development of a plan which speaks to implementation alternatives, which is client centered and measurable
  • The development of an individual plan of services and other supportive activities suitable to the client’s needs, consistent with the client’s aspirations and preferences and which maintains the client’s health, safety and independence in their home
  • Arranging for/or Coordinating those services or performing the specific services assigned to the case manager.
  • Identification and coordination of formal and informal resources necessary to meet specific client needs
  • Inter-agency and community liaison with appropriate groups and agencies in order to meet specific client needs
  • Evaluating the effects of the services and determining continuing/changing need for services
  • Maintain all records required of an Elder Care Case Manager in documenting Case Management activities performed in each client’s/member’s behalf and submitting billing to the appropriate payer source as indicated in service plan or submitting to the billing as assigned.
  • Attend and participate in appropriate community agency meetings and task forces to share specific needs that have been identified for clients/members receiving Elder Care Case Management as assigned by supervisor
  • Make in home visits as needed to ensure services are in place and needs are met
  • Assume on-call duties as assigned
  • Attend all required meetings and educational opportunities as assigned by case manager supervisor
  • Other duties as assigned by the case manager supervisor
  • Stay up-to-date on program related changes
  • Proficient in Harmony EMR

QUALIFICATIONS:

  • Dependable, patient, and manifests a care-giving attitude and fully agrees with the mission of Elder Care
  • Completion of the curriculum requirements for a RN degree in nursing, licensed practical nursing, social work, or any closely related field, and one year of professional experience in the area of aging; or any equivalent combination of education and experience
  • Excellent interpersonal and oral/written communication skills, able to work well with multi-disciplinary teams
  • Familiar with Microsoft Office software and proficient ability to use a computer and other office tools, ie copier, fax etc
  • Must be able to work independently and as a team member
  • Must have excellent time management skills and be flexible to meet the needs of the clients/members on caseload.
  • Requires travel and reliable transportation
  • Requires current Oklahoma Driver’s License
  • Requires current insurance verification
  • Requires verification of nursing license (if LPN or RN)
  • Requires verification of TB vaccination or willingness to obtain one through Elder Care on an annual basis

Pay based on experience.

Job Types: Full-time, Part-time

Expected hours: 20 – 40 per week
