Registered Nurse, Home Health Care

Elite Home Rehab and Healthcare is an accredited ACHC skilled home health agency servicing Central Indiana. We are a clinician owned agency, dedicated to providing quality care to our patients as well as helping our clinicians grow professionally. We are looking for part time and full time Registered Nurses for the Martinsville, IN area.

Duties of Position:

Provide nursing care on an intermittent basis to patients in their home in accordance with the physician’s orders. Coordinate total patient care by conducting comprehensive assessments, monitoring the patient’s condition, and coordinating services.

Position Responsibilities:

• Conduct an initial and ongoing comprehensive assessment of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Provide nursing services, treatments, and preventative procedures requiring substantial specialized skill and ordered by the physician.
• Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
• Consult with the attending physician concerning alterations of Patient Care Plans.
• Coordinate patient services and participate in the development and periodic revision of the physician’s Plan of Treatment and process change orders as needed.
• Participate in monthly in-services and case conferences for interdisciplinary care. Discuss with the supervisor problems concerning the patients and how they may best be handled.
• Participate in the patient’s discharge planning process.
• Record on a timely basis all evaluation data, treatments, and patient’s reaction to treatment and/or any changes in the patient’s condition.
• Supervise and evaluate the care given by the Home Health Aide as needed and at appropriate time points.
• Adhere to Federal, state and accreditation requirements including Medicare and Medicaid regulations.

Qualifications:

• Must have the ability to drive and access patients’ homes.
• Must be a graduate from an accredited School of Nursing.
• Must be licensed in the State of Indiana as a Registered Nurse.
• Preferred to have at least one year of experience in home health or in a hospital setting.
• Must have a criminal background check.
• Must have a current CPR certification.

Job Type: Part-time

Pay: $50.00 - $60.00 per hour
