Responsibilities
- Conducts the initial comprehensive assessment of patients upon admission to ensure a thorough understanding of their medical history, current health status, and specific needs
- Engages in ongoing and periodic assessments to consistently evaluate the evolving needs of each patient
- Monitors and evaluates the effectiveness of services provided, ensuring that they meet established standards and result in satisfactory patient care
- Tracks the progress of each patient towards their individual goals, utilizing measurable indicators to demonstrate improvement and areas needing attention
- Oversees the critical transitions for patients, carefully assessing all discharge planning needs to ensure a seamless transfer to the next phase of care, whether at home or in another facility
- BSN required
- New York RN license/registration
- One year of Hospital Care Management experience
- Care Management Certification
- PRI Certification
