General Summary
The primary responsibility of the Diabetic Educator is to provide nutritional information and resources to patients. Evaluate the patient’s nutritional status, develop, and implement nutritional care plans, assess nutrient requirements, recommend nutritional prescriptions, evaluate participant tolerance and effectiveness of nutritional care, and communicate nutritional data and information to physicians, APP’s, and other health professionals. To teach newly diagnosed diabetics insulin administration, effects, and monitoring.
DUTIES & RESPONSIBILITIES
- Evaluate nutritional status of individuals and provide nutrition counseling and consulting to individuals, as well as collaborating with health professionals within the organization.
- Plan, evaluate and conduct nutrition education programs including groups and develop educational materials.
- Collaborate with providers to identify and connect with patients in need of nutrition counseling.
- Participates in the development and planning of educational programs for groups of patients with specific nutrition-related concerns (i.e.: diabetes, hypertension, hypercholesterolemia, and weight loss)
- Participates in quality improvement measures for diabetes, BMI, and other chronic diseases.
- Is Responsible for supporting organizational mission and goal achievement, organization quality goals, and providing safe, patient-centered care.
- Assists in planning, developing, organizing, implementing, evaluating, and directing, nutritional and other educational services.
EDUCATION & EXPERIENCE
- Bachelor’s Degree Required, prefer BSN
- Diabetic Educator certification
- The ability to communicate effectively with patients, families, staff, and physicians.
- Ability to maintain confidentiality.
- Ability to perform basic computer functions.
- Ability to use knowledge of or gain knowledge of best practices, processes, laws, regulations, tools, and software within the subject matter area of this position.
- Ability to work effectively within a team environment.
- High level of expertise in the understanding of nutrition principles, theories, and processes
- Organizational skills in problem solving, decision making, and time management.
- Ability to communicate effectively both orally and in writing.
KNOWLEDGE, SKILLS & ABILITIES
- Basic knowledge of principles and practices of diet and nutrition.
- Ability to provide nutritional education and resources to patients, family members and/or caregivers considering psycho-social issues and age-appropriate, evidence-based nutritional practice guidelines.
- Ability to communicate effectively orally and in writing with patients, family and/or caregiver, and other professional healthcare providers.
- Ability to plan and organize work consisting of multiple tasks and priorities under direct supervision.
- Basic knowledge of, and ability to use, current information technology operations, software, hardware, terminology, security requirements, and capabilities.
