Clinical Pharmacist

The Clinical Pharmacist’s primary role is to provide comprehensive pharmaceutical care to the patients of the inpatient hospital setting.  The secondary focus will be medication order execution, interdisciplinary rounding, medication event follow-up and clinical pharmacy monitoring.  In addition, the clinical pharmacist will cross-cover the Antimicrobial Stewardship, Critical Care, and Emergency Room Clinical Pharmacists as needed.  The clinical pharmacist will serve as a clinical resource for medication related information and therapeutic recommendations for providers and other professional staff.  

EDUCATION AND EXPERIENCE REQUIREMENTS:

 The Clinical Pharmacist must have a Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy.  He/She must be licensed to practice pharmacy in New York State or must be eligible for New York State licensure.  Ideally, he/she should have completed a clinical pharmacy residency, or obtained equivalent inpatient hospital experience.  Active board certification as pharmacotherapy specialist, and  ACLS preferred.

PRIMARY RESPONSIBILITIES OF THE POSITION:

• The Clinical Pharmacist will cross cover the Critical Care, Emergency Room, and Infectious Disease pharmacist position as needed.
  • Verify/enter provider medication orders for the patients
  • Attend daily interdisciplinary rounds
  • Perform daily clinical monitoring for the patients
  • Improve medication decision-support relating to all areas of pharmaceutical care including selection, dose, duration and monitoring

• Participate in development, education, and communication of department and hospital initiatives/protocols/committees as applicable to the role

• Act as a liaison between the medical, nursing and pharmacy staffs in addressing questions, concerns or issues as necessary

• Participate in the preparation of reports and performance improvement projects intended to document the impact of clinical pharmacy programs on quality, safety, and cost of medication use

• Serve as a preceptor and adjunct faculty for students at the Albany College of Pharmacy and Health Sciences or other affiliated and accredited Colleges of Pharmacy

• All other duties as assigned.

PHYSICAL REQUIREMENTS:

• Should be able to push/pull 25 lbs, be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers.  Mobility requirements may include the ability to be stationary at a workstation for a prolonged period of time in addition to being able to squat or be mobile for a reasonable length of time and distance.  Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.

Salary Range:  $49.55-79.28     /hour                   Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
