What are the responsibilities and job description for the Physical Therapist position at Elmwood Nursing and Rehabilitation Center?

PURPOSE OF JOB POSITION :

The primary purpose of the position is providing and directing physical therapy services to patients which may include pediatric, adult and / or geriatric persons. The goal of helping them reach their maximum performance potential within a medical model of practice shall occur.

EDUCATION :

  • MS / DPT in Physical Therapy or BS with equivalent work experience.
  • Successful completion of State Certification Examination for Physical Therapy.
  • Assumes responsibility for ongoing continuing education and professional development
  • Must maintain valid state Physical Therapy licensure

PERFORMANCE EXPECTATIONS :

The following is a list of specific duties, which are considered to be essential functions of this job. The list is not exhaustive as all duties are subject to change.   • Provides physical therapy services to patients and initiates referrals when appropriate and in accordance with Physical Therapy Department Procedures.
  • Conducts screenings of residents at regular intervals to determine need for interventions / treatment.
  • Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
  • Obtains appropriate physicians' orders / documentation for treatment.
  • Interprets and communicates evaluation findings to patients, families, and other team members.
  • Develops treatment plan by identifying a target list, including short and long term goals and methods to achieve identified goals.
  • Coordinates, implements, and conducts treatment and plans with patients, family, and other team members.
  • Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with plan and physician orders.
  • Develops appropriate home or programming within the community to maintain and enhance the performance of the patient in their environment.
  • Willingness to work with severely physically, emotionally and cognitively compromised patients.
  • HIPAA & THE MINIMUM NECESSARY STANDARD :

    Someone performing these duties should be in good physical condition and possess the following physical capabilities, with or without reasonable accommodations.

  • Use of proper body mechanics and techniques for bearing weight.
  • Ability to sit for extended periods of time.
  • Walk through the department, office, and other locations as necessary.
  • Lift objects weighing 50-100 pounds or more.
  • Must be able to climb stairs, stoop, and reach when performing daily work.
  • Ability to assist and perform with patient transfers, (including but not limited to : stand pivot, two- person, sliding board, Hoyer lift, non-weight bearing) safely and effectively with or without mechanical devices.
