We are looking for…
A Habilitation Supervisor who can perform a wide range of duties:
Responsibilities and Duties
- Supervises and evaluates program to ensure compliance of all regulatory standards.
- Implements program policies and procedures as needed and routinely monitors for compliance.
- Coordinates and participates in provision of services.
- Coordinates referrals, admissions and discharges of Individuals in cooperation with families, referring agencies, and providers.
- Reviews and updates plans to address changes in Individuals' interests, needs and family situations.
- Ensures that staff develop obtainable individualized goals for Individuals that foster valued roles within their communities.
- Monitors Individuals satisfaction and assures that changes are made when appropriate and in a timely manner.
- Explores and oversees the development of new community opportunities that facilitate Individuals involvement.
- Routinely evaluates service delivery for effectiveness and makes recommendations as needed.
- Ensures documentation is prepared, maintained and submitted in accordance with regulatory requirements..
- Provides regular and ongoing supervision to all staff.
- Regularly conducts staff meetings to communicate policy and procedure changes, promote team building and a productive work environment.
- Performs other assigned duties as required.

Job Type: Full-time
Pay: $58,500.00 per year
