LOCATION: Embark at Vienna (Tyson's Corner)
SALARY: $55,000 - $65,000 annually
As an Embark Case Manager you will be supported by an experienced Clinical Director and Administrative Team that are all focused on the mission to facilitate lasting positive change for young people and their families at the IOP and PHP levels of care.
Case Managers focus on coordinating and advocating for services and work closely with therapists and mental health professionals and ensure clients receive the appropriate therapeutic interventions. Collaborate with the treatment team to develop treatment plans, monitor progress, and make referrals to therapy services when needed. Play a crucial role in facilitating access to therapy services by coordinating appointments, assisting with insurance, financial matters, and helping clients.
Embark operates programs across the US that support adolescents, young adults and their family’s mental health and well-being by offering a full continuum of care from Long Term Residential, Short-Term Residential to our IOP/PHP clinics to virtual outpatient care. Responsibilities
Essential Duties and Responsibilities:
- Completes education assessment upon intake and creates an Education Success Plan for clients
- Schedules weekly check ins with client
- Creates line of communication with school contact to accommodate any academic needs of client
- Tracks client attendance, including any return to school documentation
- Completes case consults with referring clinician upon admission and discharge
- Facilitates IOP and PHP sessions and scheduling
- Provides clinical support, including attendance at clinical staff meeting weekly, conflict resolution with students, and individual case management meetings with students, when needed
- Medication management coordination and support to ensure the consistent and accurate delivery of prescribed medications to students.
- Assesses clients for suicide risk and provides care and monitors symptoms for individuals with moderate or high risk of suicide
- Provides client and family discharge planning
- Facilitates pre-treatment with Enrollment Director
- Facilitates student access to health services (i.e., medical, dental, vision, etc.) through a process of fielding the initial complaint, doing assessment, and communication with the Clinical staff to schedule the appointments
- Works with students to teach and supervise household responsibilities and life skills.
- Attends treatment team meetings
Qualifications
Required Knowledge, Skills, and Abilities:
- Experience working in education and behavioral health preferred
- Proficiency in assisting the administration of suicide screens such as C-SSRS
- Effective communication skills coupled with the ability to organize, prioritize and manage schedules and deadlines
- Ability to remain in control during stressful situations and facilitate conflict resolution
- Ability to be both firm and friendly and demonstrate appropriate boundaries with students

- Remain flexible
- First Aid & CPR certified or willing to become certified
Education and Experience:
- Degree in Social Services field or combination of education and experience equal to a minimum of four years relative experience and training in youth services or mental health treatment
- Master’s Degree in related field of study strongly preferred
