Job Summary
We are seeking a compassionate and dedicated Social Worker/Social Services Professional to join our Care Management team. If you enjoy working with older adults, would like flexible hours and are ready to make a difference then Embrace is right for you. Tired of all the paperwork and lack of actual time with people? Embrace is completely client driven not dictated by insurance or government rules. Care Managers help older adults and their families to navigate the aging process; providing advocacy, education, medical management and more. Due to a continued increase in clients, we are need to add another Care Manager with a social service background to our team. A Bachelor's in Social Work or a related field and five years experience working with older adults is required. Some on call (phone response only may be required).
Job Type: Part-time
Pay: $25.00 - $27.00 per hour
Expected hours: 15 – 25 per week
