Responsibilities of a Social Services Specialist:
Assist in planning and delivering social services to meet residents' emotional and psychosocial needs.
Support residents and families with adjustments to long-term care, discharge planning, and community resources.
Maintain accurate documentation and ensure compliance with state and federal regulations.
Collaborate with interdisciplinary teams to enhance residents’ overall well-being.
Social Services Specialist Qualifications:
Experience in social services, preferably in a healthcare or long-term care setting.
Strong communication, organizational, and problem-solving skills.
