With offices across North Carolina, the care Coordinator role provides professional care, under nursing and physician supervision to patients of an assigned physician team. Provides an accurate collection of patient information to facilitate the care of patients based on a priority of need. The responsibility of the Care Coordinator is to serve as the primary point of contact for telephone and electronic patient communication.
