What are the responsibilities and job description for the Family Model Providers position at Envoi Associates?
Envoi Associates, is seeking people living in Middle Tennessee that are caring, reliable, responsible and possess a willingness to provide a safe and comfortable home to an individual with developmental and/or physical disabilities as a Family Model Provider. This person would ensure the basic physical and emotional needs required for our individuals to live a high-quality life while becoming a member of a family in their home.

Note:Family Model Providers are contracted and reimbursed at a per diem rate AND receive a monthly room & board stipend from the individual they support – this income is tax exempt.
Responsibilities include:
- Implement services and supports addressed person supported’s “Individual Support Plan” (ISP).
- Provide “person-centered” care to individual supported - honoring an individual's choice/preference, supporting them by developing opportunities to meet their social, emotional, and spiritual needs.
- Assist the person supported by attending team meetings and facilitating communication related to individual needs, response to service and support, and progress toward goals.
- Serve as an advocate for individual’s overall health, personal care, and lifestyle choices.
- Ensure individual’s privacy and dignity.
- Assist with transportation and scheduling of all medical, therapy, vocational and recreational activities.
- Follow state guidelines specific to Family Model Services. Document daily services as required. Successful completion of all required initial and ongoing training.
Position Requirements:
- HS Diploma/GED preferred.
- 18 years old or older.
- Direct Support Professional experience desired.
- Documentation of home ownership: current mortgage statement or renter’s lease
- Proof of homeowner’s or renter’s insurance or personal property insurance
- Valid Driver’s License; Reliable transportation with valid vehicle insurance
- CPR/FA certification
- Ability to pass criminal background and Registry checks as well as a Drug Screen
- TB Screenings for all members of household
- Willing to be a self-employed worker
- Successful completion of state-required training for Family-Model providers, including Relias modules and State Medication Administration training.

Pay: $24.08 - $29.00 per hour
