Job Description
The Transition of Care pharmacist coordinates care between inpatient status and discharge, provides med to bed service or referral to Home Infusion or EM Senior Care (compliance packaging) and navigates Prior Auth with providers if needed so that patients receive meds at time of discharge. TOC RPH supervises med rec technicians, inpatient pharmacist (@ admission and Discharge), coordinates counseling on new meds with patients, and provides copay estimates for patients or case managers. TOC RPH supervises TOC service with Outpatient Rx processing teams to coordinate in room delivery or pharmacy pickup.
