We are seeking a dedicated and compassionate Director of Social Services to join our leadership team. This is a meaningful opportunity to make a difference in the lives of our residents and their families.
Director of Social Services Key Responsibilities:
Oversee the Social Services department and ensure compliance with federal, state, and local regulations
Conduct psychosocial assessments and develop individualized care plans
Support residents and families through admissions, discharges, and transitions of care
Coordinate services with interdisciplinary team members and outside agencies
Provide leadership, supervision, and training for social work staff
Advocate for residents’ rights and well-being
Director of Social Services Qualifications:
Bachelor’s degree in Social Work or related field (Master’s degree preferred)
Minimum of 2 years of experience in long-term care or healthcare setting
Prior supervisory experience strongly preferred
Knowledge of MDS, care planning, and discharge planning
Strong communication, organizational, and leadership skills
Licensed Social Worker in the state of Pennsylvania (preferred, but not always required depending on facility policy)
