Job Description
- Caseload Management
- Assess clients to determine services needed or requested
- Refers clients to appropriate agencies for specialized services
- Develops action and stabilization plans with clients
- Provide ongoing case management to ensure clients remain housed
- Provide guidance and support to help clients obtain benefits (cash/non-cash) from any source.
- Assist clients with completion of various paperwork for items including, but not limited to: Rental applications, utility assistance, identification cards, applications for general relief, cash aid, SSDI application, Veterans Affairs
- Maintain complete, accurate, and timely case notes and related records in manual and electronic systems
- Complete case reviews to determine need for additional assistance or other referral services
- Work with clients to develop a transition plan for maintaining housing when assistance ends
- Maintain completion of work based on contract deadlines
- Utilizes county and company computer data processing systems to efficiently track and record client activity
- Must have the ability to work with diverse populations
- Other duties as assigned
- Bachelor’s degree in Sociology, Social Work, or related field
- Two years of equivalent work experience
- Knowledge of local housing market and applicable regulations preferred
- Proficient in Microsoft Office, databases, and other specialized software
- Strong interpersonal and communications skills
