The Life Sharing Specialist:
- Plans, organizes, and directs the provision of cross-disciplinary services to clients in a home-based program for intellectually disabled.
- Plans or participates in planning, developing, and evaluating the therapeutic care and treatment program.
- Acts as team facilitator in Interdisciplinary Team meetings to develop Individual Service Plans (ISP) for each assigned client.
- Completes monthly, quarterly, semi-annual, and annual reports on the progress of the Individual Service Plan for each client and furnishes written reports on assigned residents to designated parties as outlined in regulations.) Maintains ongoing staff training in all components of the Goal Planning/Programming. This will include formal training sessions and on-site training.
- Coordinates and implements an appropriate educational/recreational program in the daily routine of the clients.
- Functions as a liaison between parents, county personnel, educational institutions, and EHCA.
- Participates in all meetings that are related to the client and/or their care.
