Job Summary
Be a part of improving patients’ lives as a Patient Health Manager at Johnson City Family Medicine.
As the connecting thread between patients, providers, and community resources, you will ensure coordinated, quality care for patients with a variety of medical conditions. Facilitate smooth hospital discharges and continuity of care through detailed clinical assessments and communication.
Educate patients and caregivers to empower preventive health and disease self-management. Collaborate extensively with clinical teams, social services, and community partners to identify and access all needed support services for each individual.
Contribute your skills in areas like insurance coordination, database management, and clinical data analysis. Help plan and monitor the budget to ensure fiscal responsibility. Participate in quality improvement projects and research initiatives.

Knowledge, Skills and Abilities
- Knowledge of the Patient-Centered Medical Home model/mission.
- Knowledge of insurance industry practices and requirements.
- Knowledge of area ancillary services and specialty services.
- Knowledge and understanding of chronic disease and preventative care measures and their impact on patient health and well-being.
- Ability to develop educational programming for patients and healthcare professionals.
- Ability to communicate effectively and adapt communication styles.
- Ability to use proper business/medical English.
- Ability to organize, compile, and abstract data.
- Ability to participate as a contributing member of a mission-driven interdisciplinary team.
- Ability to use computer programs, including databases, spreadsheets, word processing, calendars, etc.
- Ability to function effectively within the RN Scope of Practice and to maintain active TN licensure.
- Ability to utilize electronic health records and practice management systems for job duties.
- Ability to determine own priorities and to work with little supervision.
- Ability to remain in compliance with HIPAA guidelines.
Required Qualifications
- RN;
- Bachelor’s degree in nursing (a highly qualified Associate’s-prepared RN with at least 5 years of experience may be considered);
- Must have at least 3 years of current direct patient care experience;
- Active RN License (TN or Compact Agreement).
