Reports to: Service Line Manager – Behavioral Health

Job Summary:
To provide patients with Behavioral Health needs and their families with ongoing support and coordination through comprehensive assessment, goal setting and implementation, and overall evaluation of individual patient and family needs. This position primarily focuses on addressing social determinants of health through care coordination and follow-up. The overall goal of the position is to enhance patient and family engagement in behavioral health therapy and in completing patient’s goals. Further the position focuses on the quality of patient management, satisfaction, and promotes continuity of care. Has accountability for the care, coordination, and planning of caseload. The Behavioral Health Case Manager supports the work of the patient’s provider and therapist by helping patients reach their patient-centered goals. As a member of a care team, Behavioral Health Case Managers support Evara Health’s mission, vision and goals and are quality focused to enhance patient-centered care.

Duties may include but are not limited to:
  • Provide ongoing case management to a caseload of assigned Evara Health patients
  • Screen patients for common mental health and substance abuse disorders
  • Assess patients and their family / support system for social determinants of health to create a patient-centered plan to address identified need
  • Monitor patients (in-person or virtually) for changes or complications
  • Coordinate and facilitate internal or external referrals (e.g., social services such as housing assistance, vocational rehabilitation, respite, mental health specialty care, substance abuse treatment); measured by successful linkage to service(s) with documented evidence within the patient’s record
  • Through extensive record review, reinforce clinical visit orders, medication compliance, labs, nutrition recommendations and referral follow through
  • Provide in-person or virtual face-to-face contact with patients to check-in on progress in treatment and other related goals and communicate back to PCP and therapist as appropriate, ensuring timely and accurate documentation of all patient interactions
  • Provide regular, as determined by patient and documented in patient’s record, face-to-face visits (in-person or virtual) with patients on an active Safety Plan
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the PCP and the behavioral health provider or consultant. These may include changes in medications or psychosocial treatments or appropriate referrals for additional services
  • Act as an advocate for patients and their family members within the community as needed
  • Participate in regularly scheduled caseload consultation
  • Track patient follow-up and clinical outcomes using a registry
  • Ensure safety of all patients
  • Complete a quarterly comprehensive summary on each enrolled individual
  • Build and establish trusting relationships through ongoing outreach and education thereby improving individuals physical and mental health
  • Provide case management services that are culturally competent, solution focused, strength based, recovery oriented, and co-occurring capable as they are needed by patients and family members
  • Complete all documentation per Evara Health’s standard including case records, case notes, pre- tests, post-tests, and outcome measures
  • Use appropriate crisis intervention skills, anticipates, and prevents crisis when possible
  • Report all risks to supervisor
  • Meet regularly with program manager to review cases and job performance
  • Keep accurate records of caseloads and ensures that any required measures are completed and reported to data entry in a timely manner
  • Extensive use of electronic health record (EHR) and other applicable systems for record reviews
  • Maintain reasonably regular, punctual attendance consistent with Evara Health policies, the ADA, FMLA and other federal, state, and local standards.
  • Openly, clearly, and respectfully shares and receives information, opinions and concerns and feedback in a supportive manner.
  • Work collaboratively by actively participating in huddles, team meetings, by mentoring new and existing co-workers, building bridges, and creating rapport with team members across the organization.
  • Effective and timely handling of other duties as assigned.
  • Maintain strict confidentiality as appropriate.
  • Strict adherence to all Evara Health, HIPAA, HRSA, State, Federal and accreditation agency rules, and regulations.

Education and Experience:
  • Bachelor’s degree from an accredited four-year college or university with a degree in social work, psychology, or human services related field
  • Certified Case Manager preferred but not required
