Social Services Assistant needed for Skilled Nursing Facility in Las Vegas NV
Tuesday - Saturday 9:00 - 5:30
This position works with Director of Social Services and serves as a resident advocate, working in coordination with the interdisciplinary team to identify resident needs, develop appropriate resident goals and care plan interventions. Coordinates discharge plans, locates community resources for residents and/or family members.
Qualifications
One year of experience in the provision of social services or Human Services in a long term care facility, geriatric or rehabilitation setting desired but not required.
